Paystubs? Paychecks?

Good Evening All,
I have several questions that I can't seem to find the answers to, and im hoping maybe someone here can help me.
I was working for Werner Enterprises for about a year, and then in Aug I switched over to an Owner Op who runs freight for Werner. I worked for him for 2 months total and received 2 pay checks. One for a total of $600.00 and one for $342.00. October 1st rules around and Id not recived a pay check since mid September. I call him to find out wtf is going on and am told that I was charged $545.00 by Werner for health insurance and again, I would not be receiving a pay check. So, I quit and switched back to company. Now, 2 weeks later still not recived my "final" paycheck. All I keep getting is "waiting on Werner for final to make sure advances and health insurance is all taken out". He's told me that my check should be between 0 and four hundred dollars. Now, somehow its dropped to 200 and still waiting on Werner. Ive requested paystubs and was told they dont do them. Now, by my records he owes me a hell of alot more then 400 dollars. so now to my questions-

1. Does an employer have to give an employee paystubs?
2. How can I get a record of what trips where paid to the truck?
3. How can I find out at least if he paid the taxes like he was supposed to?
4. Do i have any recourse to get what I belive is owed to me?
