If you can't find anything, I can share one.
Most places get them from a DOT consultant.
I can recommend one of those in NJ too...
Originally you had to supply an hour of training which was later modified to handing our educational materials. -
Do I need to go over it with every driver or simply hand them out? I know some companies have orientation where they go over everything and have new drivers sign stating they received and went over the material..

When a driver applys for a job he should bring a recent DMV printout. We have every driver do that and then I fax it to the ins. co. along with a big copy of his license. If the ins. co. says he/she is ok then I interview and not before. I find out the the more friendly the guy is the more he has something to hide. If he/she passes those two tests then a drug test is next. Pass all of that then it is time to look at making offer for a job. Just my.02.

You need a signed reciept for the packet in his qual file. Often it is the last page & he signs & tears it off and you keep that.
