Had a similar problem. After I replace all the u-joints, still had the problem. Then I had someone tell me to check the driveshaft. The spline in the the shaft wears eventually if not lube. Many always lube the u-joint but forget the driveshaft itself. Anyways, I replace the driveshaft, problem solved.

Problem solved. I had a manual forced regen done a few days ago thinking my dpf is clogged, truck hardly ever regens. that didn't solve anything. had a mechanic hook a lap top up to my engine and it was monitored while I drove, looking for a possibly bad injector, that showed no problems. engine is fine. Problem that made my truck shudder at speed was at rear of trans (8LL) had a lot of movement, at this point (today) it was leaking oil, I could move driveshaft up and dowm probably 1/4 inch. At shop my truck is at mechanic is going to try tightening big nut up first, he says it doesn't appear to be as tight as it should be, if slop isn't eliminated bearing will need replaced in extreme rear of trans. should be done tomorrow sometime. I'm so happy I found what was wrong.

Almost bet its carrier bearing , maybe u-joint , if its really badly worn out. You can check both of these items out yourself , if you don't mind getting a little dirty.
Hearing any noise , when you're starting out ?

There was no noticeable noise when starting out, truck would start to shudder or vibrate when accelerating. problem was large nut on end of trans that holds yoke on, it was getting loose, speedometer started acting up the day I figured problem out. parts of speedometer gear was damaged, that got replaced, new oil seal and nut tightened up. Truck runs so smooth now.

I also had a bad vibration and shaking mostly in the higher gear range and that turned out to be a broken motor mount. Just something to keep in mind. I was told Cat motors are notorious for that.
