Call and ask the doctor's office because it would depend on the way they bill it to the insurance company. I have never had Harvard Pilgrim Health Insurance but I have never had any trouble with Blue Cross paying for mine and normally the company will pay for it. If not most doctors will charge $80 - $110 for a DOT Medical Exam with card and long form. It is all tax deductible as it is required by law to have one.

Thank you, where would I get the long form for my doctor?

I did this a few years ago when i had to deal with my regular physical at the same time I needed my DOT physical, I called my doc, explained that I need a DOT physical and had her fill out the paper work under the guidance of the DOT physical form instructions. She alloted the extra time for it because it was ten minutes more than a normal physical but the problem I had was she really questioned everything and had me confirm what I was doing (taking my pills, watching my diet and so on) by providing proof - I will never have that doc do it again.

My doctor had them at his office however you can usually pick them up at a truck stop or even print them on your home PC.

I was at DMV today (Ca.) and they had the long form out on the counter.

most companies I've worked for sent you to a company provided doc. paid in full.

Starter carriers have such a high turnover they make the newbie pay for it . I had one carrier tell me I had to pay for it but if I left they'd give me the long form back .
I think $100 is high . I've seen plenty for $50 -
I've never heard of a company that makes you pay for your physical. I wouldn't work for a company like that.
