I tried looking on the fmcsa website to find what is actually needed on the application form for new drivers and couldn't find anything. Besides work history, name, address, signature, is there anything MANDATORY to have on the application? Is there any templates out there. How many of you have copied an existing application and put your company info and changed a few things?
I want to start from scratch but don't know if there are any MANDATORY things? Any help would be appreciated!

Companies will wanna know if you had any tickets, accidents and the date and or month of them.Some will ask for 3 reffences not related to you,phone and address.You don't need to treat trucking applications like resumes.All they really care about is your work history an varifying it,mvr dac.It is a good idea to keep an application for your own use so you don't have to keep looking things up.As many places as I have worked I have to keep a personal application.No way I coul remember it otherwise.

Its for my application, future employees need to fill out to work for me. Just wanted to know if there was any mandatory things that need to be on the application, for when the new entry audit takes place i can be compliant

Easiest thing would be to buy a standard driver qualification folder from a truckstop and just re-use it. It will have all the forms you need with all the right stuff on them. Or you can buy direct from Keller for about $10 and then get endless sales calls for a while.
