IMG tags not working

I've noticed lately that the IMG tags don't always work properly. It's worse when trying to get it to make an image a link with the URL tags. It seems to be getting worse with it just posting a link between the IMG tags but not putting up the image.

For some reason, I have to use the advanced function. It wasn't like this a few months ago. I used to be able to use quick reply.
