When ever you need an answer for an important question like this learn to ask the pro's "OOIDA" 800-444-5791 or something like that if you want to get the correct answer.If you rely on a word of mouth for this kind of information you can get your self in deep trouble because laws change every year and if you get outdated information you can bet your bottom dollar it will cost you plenty.

You need to call each state your going through. Should be able to find the numbers online or in your rand McNally motor carrier atlas. Each states requirements are different and depend on dimensions. Can be a huge pain. First time I tried to route a 12 wide load it took me three days

You can call a permit service and they'll take care of everything for about $15 per permit. That is what most people do.

i just bought a oversize manual from the petro in carls corner texas. haven't gone through it yet to see if it's any good. but i've been looking for a manual since i came back out. first time i seen it. i bought it. 2 1/2 years

Each state does their own. Not hard to do after you get used to it at all, and most can be ordered online and paid with a cc.
They will then email you the permit and you print it off.
If you get the list of fone numbers from the internet, it isnt that hard a deal to do yourself. You can of course go through a permit service if you dont mind paying the fee's etc. Up to 12' wide, is no big deal in most states. Some states have their little "quirks", but you will get used to them pretty quickly.
