What I carry is a black loose leaf notebook with plastic liners with my dot cab card, insurance papers, Ifta permit, apportioned vehicle registration, all state permits, I know I left out something but someone will hopefully help out. I carry a book because when asked at the coops for something I just hand them the book. Never had any problems. Comes in handy in Wyoming and other states that require you park and bring in your permits.
wulfman75 Thanks this. -
8 days of logs and your permit book .

Some states require your long form physical and your dot inspection paperwork is a must...

Although I have rarely been asked for any of these, the lease to your carrier is one item to have. Another is the written policy from your safety dept that you are relieved of duty when going on meal breaks, another item would be the road test that your present carrier completed when you hired on. Another would be the paper copy of the annual inspection.

A must is your load info (bol's , who owns the stuff your hauling) is a must in GA. held me for 2 hours in GA for not having it. they wanna know where you got it.

1. your CDL
Following should be in clear plastic sleeves in a binder or duotang:
2. proof of insurance for truck (original)
3. title/registration for truck (photocopy)
4. title/registration for trailer (photocopy)
5. log book incl up to 14 previous days depending on where you are
6. cab card (original)
7. IFTA certificate
8. MCS-90 form
9. CVOR certificate (for Canada)
10. Motor Carrier Authority (for US)
11. Safety certificate for truck
12. Safety certificate for trailer
13. Emissions certificate
14. Medical certificate
While perhaps not required, I also carry a copy of my most recent DOT inspection. I can see where it would be beneficial to have a copy of ALL your DOT inspections available for the DOT cop (if they are good that is)
Also not required, but I carry a copy of the diploma from my truck driving school, as well as the certificate from the skid pad training and my air brake certificate.
It will never hurt you to appear organized and professional.Truemac and Brettj3876 Thank this. -
I have heard the long form deal FOREVER--and Never carried it nor been asked--can someone please--all BS aside show me a reg--anywhere where it is required?

I cant yet & while I do not carry mine, its was my understanding that you are supposed to keep your long form with you. But again, I havent found the rule.
But I did find this. Anyone heard of this?

"5: What should I do with the medical examiners certificate beginning on January 30, 2014?
After you provide your State Driver Licensing Agency (SDLA) and your employer with the medical examiners certificate, the medical examiners certificate will only be valid for the first 15 days after it was issued. Your medical examiners certificate will have been recorded on your SDLA driving record and your driving record will then become the method of validating your medical certification."
