Medical records question

I had a stroke 3 years ago this passed March. I made a full recovery in the emergency room. My doctor has cleared me. The stroke was caused by a small piece of cholesterol in my carotid artery. All of my tests showed me to be completely clear in all arteries except that one small piece that was in the left side of my neck....A year later an ultra sound showed that piece to be completely gone. I asked my doctor then if I was ok to drive a semi and he said yes....Should I get a statement from my doctor to this affect.....All reports came back using words like "remarkable" to expressing how clean and clear my arteries are.......No other blockages or build ups of any kind anywhere........I changed my diet majorly after this happened. Gave up cooking with lard , started eating more grain type bread etc. I am completely healthy other than that one incident.

My company is telling me I need to do a sleep study at their doctors. I do not want to do it and am thinking of leaving them but will that follow me around on my DAC?
