Here ya go, straight from the FMCSA.

You're required to declare all commercial driving jobs within the last 10 years when apply for a driving job.

    See 391.21 at http://www.fmcsa.dot.gov/regulations/title49/section/391.21

Parts (b), (b)(10), and (b)(11) cover the applicants requirement for disclosing all commercial driving jobs over the last 10 years.

Welcome to the zero defect environment. And computers never forget anything.
Another reason to get a job on the docks at Kroger.

a lot of the information you OTR guys are posting does not apply to local jobs. Local is completely differant than OTR and has differant rules and reports

And to the OP you can get a class B freight job, but expect to go to very tight places. Freight companies run some straight trucks because they send them to places that a semi can not get to.

    ^^^^^^^

THIS! Very well said, and VERY sage advice IMO. I've been driving big trucks over 15 years (the last 2 exclusively oversize/overweight) and I will do anything I can within the law to stay the he77 out of NYC!

Nope your not screwed, just start applying to other companies NOW and tell them the truth, you were in an accident where you got rear ended and you got fired for it. Don't mention anything else, it isn't important.

By the way, outside of the bus mirror, I wouldn't even bother reporting things like mirrors to the police, it isn't worth it. I was doing a Ohio to Conn run for a few months 5 years ago and bought four mirrors that I kept in the truck because no matter what coming off the GWB with any truck next to me I would always lose my right mirror. I got good in changing it in less than 10 minutes once I got off the freeway.

Glad I read thru- this is exactly what I was going to post!

Like Generic says- get copies of the police reports, ESPECIALLY the rear end and hit and run; that way you can show any prospective employers you were NOT at fault. That just leaves your mirror hit, which in the grand scheme of things is not that big of a deal really. Good luck driver.
And seriously, NYC sucks! Get out of there and find some wide open spaces!

I had to report it. Im in a company vehicle and so is the other guy. Last thing I want is for him to claim a hit and run later. The hit and runon me I had to report too because they would have seen it. Maybe they wouldnt have noticed it right away and later they wouldnt know which driver did it but I guess im too honest for my own good.

Im going to get the reports on monday along with a new MVR. The mirror I broke was my third day of driving proffesionally and I was thrown right in to Manhattan. If an employer holds that against me then that is probably not somebody I want to work for.
