    Whats the form 3pg for the medical card? The doctor is messing me up on it cause they didn't give me every thing i needed so i gotta go back today and pick it up

    I had to cancel my checking account Last week cause it got hacked into so i just opened a new one and haven't got checks yet i hope i can just give them the info instead of a voided check

    my medical form was a front and back as well as the card..I don't think it's 3 pages..how much are those motor carrier atlas books??

    From what I've seen online going rate seems to be 100-125. I was surprised to only pay 80. And mine is 3 pages.

    Mine was $137. There are a few private doctors that do it for cheaper but I didn't want to wait, CVS was convenient. That's what I paid for. Lol.

    I paid 60 for mine and wish i woulda paid 110 cause the place i got mine has been nothing but problems but you live and learn and also i thought the medical card was like a business card mine is small piece of paper i dont know how im ganna keep it from getting messed up for 2 years

    You should have a 3 page long form, that has all the boxes and questions and info the doctor and you go through, and the results of all the test. Then you should have an actual card, mine is a long rectangle front and back that folds in half, some are just front and back business card styles. The school should be able to laminate it for you I know trenton does, or if you are near a ups or fedex store they can laminate it for better protection.

    atlas is 15ish at T/A or Petros for non deluxe paper atlas, and 30 for the deluxe laminated atlas for 2015. you may be able to find the previous year clearanced for a much cheaper price.
