Ever try cutting the grease with some diesel fuel in cold weather? I heard the guys up in northern Manitoba do that in the winter and it apparently works alright.

We're all human. So long as you catch the mistake before it becomes a bigger problem, it's all good.
I've left trailer box doors open a few times over the past year and had to stop on the side of the road to close them. I almost drove off from a pickup yesterday without re-inflating the trailer suspension. Among other things. -

I learned it's not enough that mudflaps are present. How they're attached is important. Some driver had reattached a trailer flap with nylon rope. During my trip the rope wore through. The flap got sucked in between the tires of the rear tandem where it rubbed on the tire inflation line until it failed. When I stopped for a break and did a walk around, there was the flap wedged between the tires, hanging from the remaining piece of rope. Glad I didn't lose a tire. Make sure the flaps aren't just there but firmly attached. I carry rebar tying wire for when I rip off a flap. It happens.

I did the same thing. New Freightliner has a tether on the windshield washer cap but not the coolant reservoir!!!
I only got two miles before truck wanted a regen. During the regen I noticed a coolant stream. Just like yours the cap was still in the spot I placed it!
Now I have acquired a spare fuel, refr and coolant cap. But the fuel and coolant were for my old volvo... not my current Freightliner.

This happened early in my career. I was not doing nothing but looking at tires. I was doing a post trip only. That night I learned to do a walk around and check lights. Also no I never used a CB I listened to XM radio.
