Run away from them, but first refuse to sign the paper and get fired. State labor board will have fun with them.

Run, do not walk, away from these guys.
1. You should not have to pay for your own workers comp coverage. Per every states law, the employer is required to furnish that coverage.
2. You are misclassified as an independent contractor. You are an employee per irs regulations.
This company is skirting the law. -
Sounds like risinger.they did have you pay 200.00 a month for occupational insurance.it was mandatory.they do pay 1099

I'd really like to know what this fella's pay rate is. Paid on 1099, driving a company truck, no benefits and now they want him to pay his own insurance.
That 30 cents a mile won;t go far. -
I get paid 32%of what we make a week
I work a flat bed
The 32% is me driving and my wife as the dispatch
