Here, on this side of the fence, skidded tires are considered as a preventable incident and you're losing the bonus. No matter if it was frozen or corrosion.
If you get hit by somebody while you were legally parked or moving, it should be considered as non preventable.
Running down the road at 65 mph with an empty trailer on a windy day and getting blown off the road by an act of God might be ^^^^^^ the call of a comitee.

I had a green light at the bottom of the off ramp green at the top scan for traffic and here comes a sports car running the red light I missed by an inch I think I could barely stand when I got back to the shop.

Honestly? That's not a guranteed preventable. That's how messed up this system is. Your safety guy could argue you didn't "count to 3" before you moved, or blah blah blah. That's why determining what is and isn't a preventable is futile. It all depends on what the safety czar (as nice as I can put it) decides that day.
It's not right, but basically if the safety guy says it's a preventable you're gonna have a hard time proving otherwise. -
Still preventable. Bridge inspectors could have seen a problem and closed the bridge....
