Never have paid for one. Employers in over 4o years total , have always paid. Only two really if you don't count working for family. If you do then 3 employers.

I acually had my physical done at CVS. It was more expensive that where I went in the past.
Here's why, I was home on hometime. I 'forgot' to make an appt. at my regular doctor and they couldn't fit me in. I wanted to get it done without hunting around out on the road. You pay for convience. Just like Jiffy Lube...
The CVS is walk in. I waited about 5 minutes and then it was my turn. The person doing the physical was a PA, not a Doctor. With the DOT rules now, she took the time to get certified. She was very professional. VERY thorough, and really set me at ease. She took about an HR to do the physical.
While it cost more, I was reimbursed from the company. I was able to then go to DMV and make sure I was self certified and there was no issue. The extra $40 bucks from where my last physical was done was worth it for the no appt, no wait, relaxed at hometime, and right down the street from the DMV.
I've wasted $40 on much stupider things....

And as far as I know, all she did was dip the urine test...

'Insane' pricing? Lol a bit strong...

Have never paid for a DOT physical yet. The companies that I run for have always picked up the tab.

The dip test checks dont just test for sugar, it usually tests for 5 different common health issues like urinary track infections, diabetic ketoacidosis, high bilirubin count, and high levels of potassium. All which can be signs of a serious problem.

On a side note, my family dr charges $160 for his dot physicals including drug testing

I've just been going to the clinic. $60. If i have to do it myself.

As for the urine. I've been told that it gets sent out. And that's NOT just the clinics that do that. I guess there's a laboratory that does the urine tests.

Lab corp sounds familiar, If it still goes by that name. Just about everything around here has new names now. I did my prehire 3 months ago at some name i never heard of. Didn't even know there was a clinic there.
