For question 1...
If you’re a sol-proprietor I don’t think it matters what type of bank account you have. If you are setup as a cooperation a business bank account is what you would want.
Yes, you will want to invoice direct customers (at least if you want to get paid
).
An invoice is an invoice, no special type for freight that know of. There are some trucking software packages out there that you can use for generating invoices. Trucker Pro is one popular one. You can download a demo of it and use it as a guide for creating your own invoices if you want.
For question 2....

Ubu has a pretty good post about that infamous qualification file and what specifically is required of drivers working as a driver to a Owner Operator. I have had been in that situation once or twice and needed to be familiar with that file.
