I have a 2007 9400i it has a Cummins isx
450 in it. It has started to make a really loud noise when under a load. I thought it was in the clutch so I had a new clutch, stop brake pilot bearing and input shaft put in it. Still no change. It does not make the noise while in lower gears when I hit the top side of the tranny it starts making a really loud rattle. It quits as soon as I get off the throttle. Not sure what it could be.

Where is the noise coming from? Mine does it but it's the clutch peddle. Put your foot on it when the noise starts see if it stops.

Hole in exhaust manifold or manifold gasket. Classic symptom.
