You guys are smart enough to know when it is too soon to go. After all nobody knows a truck better than it's owner. If she hiccups or is not giving full go, clearly it needs longer warm up. Newer commonrail engines with thinner oils are much better at tolerating the colder climates up here in cold north as far as cold runability. That brings another point. If your not running 10/30, that can make all the difference this time of year.

For the idle for 5 minutes and then shut down, it is not an epa item. It is a setting in the ecm that any dealer can change. It is "idle shutdown" and it can be turned off. Way up north they do high idle when ever they get out of the truck. They set them to 1200 rpm. The high idle speed can be changed with insite if you have the correct version of it.

As for the DEF, Goodysnap pretty much covered everything except that when it gets so cold the system cannot keep the fluid in a non frozen form. After a certain negative temp the EPA says that the engine manufacture is no longer liable for the DEF system to function properly. In short once it gets so cold the EPA says that the emissions device no longer has to function because it won't be able too.

As for the 5 min idle time on mine, yes it could be changed, I tried at a Pete dealer in Pasco wa, but it required a password which I don't know and they didn't either....said it needed to get it from cummins. Which I wasn't going to wait for.
