Generally by crane or forklift, but there are times a flatbedder will hit a dock like a doorswinger. I do it quite often, myself. Nature of the specific freight I haul.

Usually forklifts off the ground.

Sometimes you might back to a dock and be loaded like a van trailer. And there are docks where you back into a slot where there may only be be a few inches on each side, the entire 48 feet, and your entire trailer can be accessed from dock height, either of the 3 sides, and they typically load from the sides.
