Anything under the feet puts it over height, it rode from la to van horn tx with no issues. Why its suddenly having problems no I couldn't tell you. And yes I hate this trailer. It's been welded multiple times. And yes it should probably have been loaded at the front. Six was saying it needed 2 of the feet off to allow it to flex because it is more rigid than the trailer. And 15k is correct on weight.

Through the rail and deck then back around the rail the hook would hook to the top of the rail. And the strap will wrap under. That way there is no side pressure on the hook. That is the weak point.

ok from what you have so far i would of hooked you hooks on the rub rail then brought strap up thru the pocket. the way im suggesting is take your hand palm facing you reach into the pocket inside rub rail hang fingers so your nails are hanging over edge of rail facing you. ( that is the way i would of done the hook) the strap will come out at you then cover your edge of hook and head upwards to your load.
what i see, you have no point of contact from your load above an axle. how heavy did you say 15k .......is what my head is remembering. Im seeing it as your dunnage is bouncing load between axles up and down. causing the vibration to tear and weaken your hooks. how does this ride down the road? do you have any hop or major bouncing?

It is most likely the flex of the trailer loading up the side of the hook. Kinda like bending a coke can tab. Enough times and it pops

I'll move the hooks around. I've always been Leary of hooking to the rail because when I got this trailer it was busted loose in a few spots and I stuck the bar in once and tried to bend it out to feed a hook through and the rail broke loose just by me pulling on the bar.

ya can break the pocket welds real easy with a winch bar ive done it also. but i still hook to the rail.
