The 11 digit trim code file is on the disc that came in the box with the injector, you put the trim code in with Cat ET whe you change the injector. these are callibration codes are to calibrate the injector for emission mostly. not putting it in wont give you a misfire or any smoke, may make it idle slightly rough though.
Leaky injector smoke is usually refered as white smoke, if thats the case please go through what you did when when you changed the injector, was the engine warm then? you said it only misses when its cold yes? how long does it have to sit for to get cold enough to miss?
If it has a dripping injector tip when sitting on another cylinder but you drained the fuel out while you changed the injector there would have been no fuel in the rail to drip in that time. I really doesn't sound like an injector fault to me, if it smokes until its warmed up everytime its cold then it sounds like a lack of compression on that cylinder to me.

It only has four digit injector codes. Yes the engine was cold after i put the injector in. It acts like the injector is just pouring the fuel in and it wont burn it until the engine temp raises.

Pull #3&4 injectors and stick a long screw driver on top of the piston. barr the engine till #3&4 is up. see if #4 is coming to the top, may have a bent rod.

Bent rods on Cats = windowed blocks. They don't just bend, they kill everything in reach!! Heavy iron there...

Got curious after reading last few posts, and Jas's comment about the injector #s changing from 10R0957 to 10R8501; have read about the injectors # 57, 58, and 59. Since one of mine was recently replaced on my '04 MBN, I pulled the invoice and noticed the injector # for my replacement = 10R1000. Wondering if that is comparable to a 57 ?

Not necessarily. I have changed several that did not ventalate the block. Change one just a coulpe of weaks ago, ran rough and smoke white real bad till eng temp came up. Had a bent rod on #4. Replaced cyl pack and it ran like a top.
