PSP vs MVR

So we started running PSP's prior to getting drivers cleared by insurance etc to prevent the misconception that we have a solid driver being onboarded only to find out that our insurance company ran his MVR and labeled them with a bad score which will affect our renewal if we added them. Till now I was led to believe that the PSP record has all violations, roadside inspections for all licenses the driver has ever had combined. We ran a total of five, two of which came back with roadside inspections with no violations at all(one from three years ago), but the insurance scored them with the worst possible score they could be insured with, how is this even possible? I don't see how something wont show on a PSP but will on an MVR, I would think its the other way around? At $10 a piece and constant audits on these records I would expect them to be accurate...

Based on that, lets say a driver had an at fault accident with no injury's/death's what is the typical fall off point for something major like this(after being convicted)? is there a chart to reference? maybe these drivers have violations going back further than what I can see

 

And just to confirm any and all driving violations accumulated outside of the truck will impact the CMV license the same as if it was a CMV ticket, or are they weighted a little different?

 

Most of the confusion came from line three on the PSP "always includes original violation regardless of whether it resulted in a different conviction" I assumed this included speeding tickets etc, I do not have a CDL therefore some of this is foreign to me
