Update from driver, the injector and exhaust push tubes failed, he thinks he set the injector incorrectly and went to start it up and it broke in half, said it was pushed up against the exhaust push tube hard. Hes working on pulling the followers to check the condition of the cam and to pull the remaining lower half of the injector push tube. Any tips on getting the 1-2 cam follower out?

Once it’s off, pull out all the push tubes, thens it’s just 6 bolts. And a prybar to work the rocker cover off its dowls.SpeedyGonzalez Thanks this. -
Quick update, currently tailing the driver on the way to the shop. Holy crap the carnage, exhaust and injector push tubes for cylinder 2 were in pieces with the bottom of the exhaust suspected to be in the pan. Cylinder 1 injector pushtube was bent as well, removed and replaced. I suspect that this has been the cause if the knock all along as the cylinder 2 knock us gone and cylinder 3 has an apparent knock, push tubes all around I guess. Drivers limping it home the 150ish miles and I'm pulling the followers tommorow in the PM.
Edit: Truck is in the shop with no new windows in the block or loud clanks. I've got a feeling the issue has been bent pushrods this whole time, I'll be pulling the rest tommorow to make sure as well as dropping the pan to find the missing piece.Last edited: Sep 12, 2019

Dropped the pan to see this on the number 3 injector lobe, looks like something got caught between the follower and cam. I cant see anything embedded in the cam and I dont think it's made it through the hardened layer because it isn't flaking, run it or replace it?

As promised, pictures of the bent pushtube and ensuing carnage.
This little guy spent 200 miles in the oil pan after what looks like being sheared in two by the broken injectector push tube and then wriggling it's way behind the follower. Strangely enough, the rocker end of it is literally split in two. -

The tip of the cam is where are the cool stuff happens. It’s hard to tell from the glare on your photo. It may have just got scuffed a bit as the parts that wanted into the pan made their way that direction. But if theirs any damage to the tip of the lobe or any scratches on the cam follower roller. Your better off getting it replaced now. It won’t take long for a scratch on the roller to exponentially multiply!!!
