Driver qualifation file

    So I'm putting together my DQF. I have my app, mvr, med cert, pre employment drug test. I have a couple questions In the fmsca there a portion saying I have to do a physical doctor test? And a road test? So do I actually give myself a road test? Also how do I go about getting my records from my last 3 years of employers? Do I just call them?

    Yes you give yourself a road test.
    Now you are the employer(mc) and employee.

    Your previous employment records. Yes you can call, email, text any type of request is good.
    Keep a copy of your request in your records.
    You also need a statement by the driver(you) of all tickets received in the last year. If none: state none.

    You do not have to have records of your pre employment, just swear you verified it, which in your case means nothing.
    You had probably better give your self a road test .lol
    I have never had anyone give me a road test, since my very first job in a big truck. lol

    Order a J.J. Keller driver qualification folder. It has everything you need. I got mine from OOIDA, I think. OOIDA also has some free educational videos about all the files needed, which goes beyond the driver qualification folder.

    Call their human resources department and ask them how you can get both a drug/alcohol and employment verification. This will be done either through fax or email. For the drug test, you will need to send a “Release of Information” form. I also sent a short letter asking for employment verification between so-and-so dates.

    What does a guy do when he has basically worked for himself, under his own authority all his life.
    I always have, but also have drove for other people short term on some jobs. When I have driven for somebody else and had to fill out the paper work, I never list the of the others I have driven for, and everybody that I have driven for knows I have driven for others, they have never said a word about it. I tend to think it is mostly just a formality to have in the files when they get audited.

    A few months ago, I actually leased out, so they have to keep a file on me, I just put my company on the list and they know I have driven for others short term, but it was ok.
    Now, they did not call me either to verify, I had been employed by me either.

    They audited a couple of relatives.
    Fined the company because they didn't have a driver application on file. One was president & other vice president & both held a CDL. Neither drove trucks for years lol. $2500 fines

    In terms of the road test, I have learned that some inspectors will ask for it and some won't. Its better to be safe than sorry. You cant give yourself a road test but if you know of someone who has a CDL (perhaps even the dealer that sold you your truck), he/she can give you a road test.
    The doctor test is for the medical card. YOu need to be considered medically fit to drive. Make sure your medical officer is on the national registry!
    For the employers, yes, its called the Safety History Records Request. You can make 3-4 attempts to get them to fill it out (you can find it in google). Keep records of all the times you contacted your previous employers just in case they don't reply.
