The Inside rim was the problem yes you have to tighten that one first then you put the outside dual on and tighten it also there are left and right turns these are special you have to have the right tools and knowledge to do this

If it was hub piloted wheels it can happen. The guides on the hub are worn and let the wheels move. Was told by a tire shop not matter how tight you tighten them down, the rims will move and back the nuts off
