If you are leased to a company, then yes you must have "operated by" above the company name. City and State are not required. FMCSR 390.21(b)(3)

Only if there is another name on the truck.
Code 390.21(b)(3)
(3) If the name of any person other than the operating carrier appears on the CMV, the name of the operating carrier must be followed by the information required by paragraphs (b)(1), and (2) of this section, and be preceded by the words “operated by.”Rideandrepair Thanks this. -
I swear, if I asked what time it is , some of you would tell me how to build a clock,
it so simple, two names , two extra words.Ubu Thanks this. -
Let’s not forget some states have their own requirements also.

Most states only require the federal minimum, but a few have their own requirements for signage.

Last time I was leased onto a company. Owner and Operated above my name and city on the door.
On tool box doors was Lease To: ABC co. Followed by required info.
I allways made thier name smaller but legal. Just to make a point on who makes the decisions. -
The last few years they’ve had a bug up their butt about Leased To and Operated By. It’s just one more thing they can bother you about during an inspection.jamespmack Thanks this.
