ISX's were fine around 1350. Granted, you had to shift a little more often.

    2019 565/2050 41k miles, 3.25 rears.

Milage is fairly consistent at 5.8 to 5.9 running 65 to 70k gross. It is a bit better when I am not running the hills and and against the winds of Wyoming.

Had what seemed to be an engine vibration when pulling hard right at 1300 rpm. Dealership reset the overhead and that helped the vibrations substantially, but not completely. Apparently there was a TSB from Cummins on this.

Other than the vibration, I have not had any issues. I do agree with others here that there is no way this engine likes to be down at 11500 rpms. Mine likes to live 1450 to 1550.

How fast are you going and what kind of truck? (I'm guessing a classic.)

Hey guys figured I'd give an update. 141,804 on the clock now. Just had the TSB done for the little EGR/exhaust piece that's been troublesome. Other than that and the fuel filters that need changed OFTEN it's been pretty reliable. I cant say what mileage is because I don't check it.. typically not getting all that great pulling these loads anyway. I can run all day on the fuel I pack (270 gals) weighing 140-150k. The engine does vibrate a lot at low idle. I dont idle hardly at all and when I do its elevated and seems to be quite a bit smoother. To date the emissions system has operated flawlessly- probably just jinxed it but that's the way it goes. Not engine related but at about 80k had to have the front driver rebuilt. Input shaft lash was too tight and it started to eat itself. Warranty took care of it. Good luck everyone.

Ask for a primary fuel filter with a larger micron. I believe it's about 30 microns. I've been using those filter for over a year now and they last the 20k miles for a PM. the original filters only lasted 6k.
Also, ask them to set the ecm idle to 650 rpms, idles much smoother. 600 is rough.

Where did you take it to get the parameters fixed I have 2019 389 565hp 1850tq 342rears 18spd she pulls great. In flatbed I was averaging 5.8 to 6.3 now I'm doing reefer and its low like around 5 to 5.2 on heavy loads but I was thinking also my mpg was down cause of the weather different fuel blend but if you're getting 6 to 6.4 on reefer that's great cause of the wind drag of the trailer

I idle at 850rpm engine sounds smoothly then I wouldn't idle so low with this dpf system I sometimes idle at 950 but that's usually when it's really cold just to keep the exhaust filters good
