The cam was soft I've been told in older ISX. I don't exactly know what the piston issue was. And in reality if you do the recommended Cummins linehaul use maintenance intervals you are doomed. 30,000 mile oil change seems stupid. And 500,000 miles overhead is insane. That is most like the main issue. Also the low Cummins oil pressure does not seem to be the best thing for ISX. But you can shim the oil pump for better pressure, add gear lube because it's better than the Lucas junk. And 100,000 mile overhead, and 15,000 mile oil would be a good way to get over the million mark. And still be running like a Swiss watch.

Most of the comments are bang on! The CM570 (1st generation ISX) was a good engine, but yes it also has it's problems like: eating cams (especially the valve cam), dropped liners, and oil leaks. Like other posts suggest: Change the oil frequently! Use a bypass oil filter system with 1/2 gal of light gear oil added at PM, to thicken and add oil pressure during hot summer driving. Unless you shim the oil pump, the ISX doesn't produce enough oil pressure to use a centrifuge/spinner type filter. Practice more frequent top-end sets than the recommended 500,000mile by Cummins. Install a pyrometer if you don't already, and don't overheat this engine!
