How does the smoke test work for exhaust leaks?

There are so many things that can cause frequent regens, it is really frustrating to troubleshoot. If the engine is healthy, you probably need a new DOC. There is a comprehensive after treatment diagnostic manual for us to follow. They will inspect the truck and engine for any obvious problems then check to make sure the air management parts are working, then do a road test and record a 0-60 performance snapshot to make sure the engine is producing proper boost, fuel and exhaust pressures. If everything is ok there then they will focus on the aftertreatment. DOC's do not last forever. They do wear out and simply do not produce the proper temps across the whole face of the DOC to do a proper regen. This typically happens around 6000 hours.

If they do a forced regen it will smoke through the pipe, easy to see.

The pre DOC is just a smaller DOC right after the turbo to help increase temps during a regen. If you still have he original DOC I bet it is just worn out. There are aftermarket makers. Road Warrior is one brand we use all the time.

Just stopped to take a break, and investigate a noise on the passenger side, the wastegate actuator has no bushing in top hole, it's just shaking around, could that cause wastegate to stay partially open?

If the actuator is not secure it could keep the gate partially open. Not sure what you are talking about regarding the engine brake actuator?
