A friend had this happen to him after getting a head done at 1 million km. Went 40k and spat #2 crown skirt and liner into the sump. Injectors replaced, new liner pack. 300km up the road heavy blowby. Bore scope and cylinder #2 liner had mirror finish. Turned out it had a wiring issue in the internal harness getting an electrical signal from another injector. Ecm, internal or external harness at a guess.
