I'm familiar with that. I had to renew my DoT physical around August of 2015, and I remember receiving that letter in the mail about certifying shortly before it was supposed to expire. If I decided not to renew my Medical card, then my license would of been downgraded to just a regular class C, but I got a new physical card, faxed the MD MVA a copy of it along with that paper they sent me.
4 year old, unused CDL with no experience. Any chances?

Go to the DMV, if you have a medical card to go with the CDL, you ask for a self certify form. On that form is one through 4 levels or kinds of trucking. For example you swear and sign to Tier one for interstate and in state commerce full trucking. As long you have a medical card.
For me in particular I signed Tier two for years. The revenuer told me that is not required to hold a medical card at all, and it's grandfathered under Arkansas Law. Provided I self certify each year. Arkansas is not allowed to certify. Tier two is as explained to me essentially a resource to Uncle Sam or to the state in terms of a natural disaster or enemy nuclear attack, I would be hauling mortary in a reefer truck.
I turned in my CDL some years ago beause I no longer have a use for it in light of new medical diagnoses. I am literally done here. But that's ok. There is no problem. -
Hmm okay, guess I'll stop by the DMV tomorrow just to make sure I have that self-certify thing done correctly.

If it turns out that you no longer have a CDL, just go out and look for any job to get a paycheck and time on an employer's payroll. You need to keep up your work history.
Afterwards, work like a son of a gun to save money for either a refresher truck driving course, or perhaps going through CDL school all over again.
If you are single with no children, you should be able to go OTR to all 48 states and Canada. (Be sure to apply for and receive a passport).
If you are married with children, you may want to stick with either LTL or dedicated OTR which gives you more home time.

That's the fun part... I get a letter in the mail a few MONTHS before my CDL is up for review / renewal, to REMIND me to keep my DOT and Hazmat current. Yes, I always do because I'm still driving, but I could see someone "missing" that important piece of info, because it's not really lumped in with the DMV requirements for the license itself. Yes I kept my DoT medical card valid, I've renewed it once since I got my license when I sent in a copy of it along with my DL and that form that looks exactly like that certify form. Still is current too, expires August of this year. If I find out my license got downgraded and I have to go through school again, holy cow am I going to be livid.
