Hello! Firstly, I live in Marion, Indiana. This is my first time doing this, and my situation is a little unique, so please bear with me.
We will start this out with some deal breakers:
If you offer BC/BS, please do not reply here. My wife is sick, and BC/BS has refused to pay her visits to specialists and surgeries even though they were in-network, so I refuse to pay any more money to them for nothing.
I will not drive a truck that has a camera inside the cab. I believe this is an invasion of personal space, regardless of any safety rhetoric. I do nothing illegal or have anything to hide, I just believe in my personal privacy.
My situation:
Currently, I'm a flatbed trucker. I have 6 years of OTR, with only a 3 month gap in my driving history due to personal loss. I have no moving violations, and the only incidents on my DAC/MVR are all over 3 years old and minor (the most serious one is failure to obey sign when I started driving and missed a weigh station that was in a Rest Area just south of Pittsburgh, PA). I've been flatbedding for just over 2 years and love it, however my back is breaking down and due to medical advice, I'm seeking something away from flatbed (sadly). I love the company I work for, and have a great gig, but I have to leave. Here is the breakdown of what I currently have, and would like to find something as close as possible.

What I'm looking for:
Preferably I'd like another guarantee pay gig, as I've grown accustomed to having a dependable paycheck (if your company has the freight to keep me running, with hometime [see blow] and you can show me drivers with you are taking home $1k /wk +bonuses I'm willing to talk)
I can haul dry van, reefer, and tanker (I have tanker endorsement, no haz-mat as I'm still a smoker although trying to quit)
Paper logs are preferable (negotiable)
Running lanes - I'm willing to run any state except NW, and NE (CA, NJ, NY, PA, VT, ME, MA, etc). I have no problem running any central state, or VA and south on the east coast.
Pre-pass & EZ-pass (or I-pass, something where I'm not paying tolls and waiting for reiumbursement)
2014 or newer equipment, preferably automatic (i've gotten used to it) [negotiable]
APU w/ power inverter a must!
XM radio [negotiable]
About hometime: This is negotiable, but it's my ideal.. Right now I'm home every weekend, but the times I get home makes it virtually impossible to get in to the doctor, and it's usually a 3 month wait before my current company will get me the time I need. I would prefer to be home every weekend, so that I can take care of my wife, however I know that the deal I have right now is unique. I'm willing to stay out AT MOST 14 days.
I understand that this all sounds very arrogant, but it's really not. I fell into my current job, never intending to leave them, but I must. I have to juggle work with a sick wife, and cannot afford crappy insurance that will just put me further in medical debt than I already am, nor am I willing to sacrifice my remaning time with her to drive someone's truck while I make pennies on the dollar. She is unable to do anything for herself, so all housework falls on me. I'm a dependable driver, never complain about the runs I'm given, never late because of something in my control (the only 2 times in 6 years I've been late were both because of weather), and ask for very little except to be paid fairly for the work I do and to get home to take care of my wife.
I realize this has been a very long and 'different' post from what others usually post here, and I apologize for that. I know what I need to survive and pay my bills, so that is what I've posted above. Thank you for your time and consideration.
