99' w900. 2ws cat. Showing a code 42?

I've ran this truck for around 8mths and the code has been there the whole time. It doesn't show until the engine has been running for around 15-20min. The engine temp doesn't seem to matter.
The truck has many problems and many miles but seems to still be fairly strong. It is a factory set 500hp with a few hot codes. Any help would be appreciated.

Could be gear lash is off on cam. Has the engine ever been worked on ? Like the head being removed.

Cam timing is slightly out. Needs the idler retimed/ adjusted in the front end
