If I attempted to list all of my screw-ups and boo-boos over the years, the internet would run out of space! Biggest factor for me has always been working (or driving) tired. And it sounds to me like the OP was tired. Tired, and stressed, and in a hurry cause he's feeling pushed.....it sucks.
OP, crap happens. I know, cause it usually happens to me. You must've caught the crap that missed me (I thought I was having an incredibly good day not too long ago! Sorry).
As a heads up, Super Service does have some pretty good drivers and trainers. Not the greatest company, from what I've heard, but not a bad place to start over. Keep your head, GET YOUR REST, and pay attention to everything (trainer, surroundings, what you're doing), and RELAX. Eventually, you'll have the confidence born of actually knowing what you are doing and be enjoying the view out the windshield.

"Lucky" is accurate. In many fewer than 22 years, I've had: a bird shatter my windshield twice (two different birds obviously...not some angry bird stalking me around), an impact with a deer that smashed in the left quarter fender and punctured the steer tire, a Roehl driver smash in that same corner of the truck right after fueling while I was on foot going to the fuel desk, a guy at night walking along the southbound shoulder of US 82 on the south side of Montgomery AL pick up and throw a large rock straight through my windshield on the passenger side, spraying tiny pieces of safety glass all over the inside of the cab (including on me but thankfully I didn't get hurt, apart from being slightly traumatized by the event), quite a few tires blow out (sometimes causing damage to the trailer), a trailer door come off when I opened it (actually the bottom hinge held...to get the door off I had to hook a rope to it and tear it off using the tractor), tumbled freight in the trailer (one time a bunch of Anheuser-Busch kegs full of beer tumbled in the rear because they weren't preloaded properly by the shipper), not to mention a few minor preventables as well.OPUS 7 Thanks this.

I didn't get any training. I hauled 2 loads of sand with the owner of the company, grinding gears and all. I was given a day unladen, by myself, just to practice shifting. Then, I was given a flatbed and ran 2 days following another truck. After that, I was all on my own. It was not easy, but it did not intimidate me. I have had a few incidents, but nothing drastic.
