You have to be honest that you cannot find this guy you used to work for. They could easily determine you do not have a DOT number of your own on a background check.

Dude, I will tell you when it comes to this crap, a lot of us have seen all that can be seen.
I don't get why you would give a crap if you can get in touch with the O/O you worked for, that's not your problem. Put his contact info down and if they have a problem with it, tell them that's what it is.
Using a "LLC" will get you canned with people like me and it is falsified app if it is use which means with people like me, I would fire you on the spot where ever you are at for that.

Some companies might take your word,but a lot of other companies will only take verifiable information the key word is verified.If a company can’t verify they won’t count it .Some companies will not even except information from an O/O if he doesn’t have a real business address and public businesses phone in the company name.

If you’re truly an LLC and pay yourself than just call a payroll company like ADP (there are literally thousands to choose from) and have them handle your payroll. You give them all the info and they’ll call every week, or however often you choose to be paid and ask you if you’re filing payroll.
If you’re getting a check that week they’ll file all the necessary taxes and send you a paycheck complete with a pay stub showing all of your deductions. They all do direct deposit as well. For $20 you get all your taxes filed, proof of income and peace of mind that you’re not falling behind with the IRS -
Let's not forget the fact that nobody will be running their own LLC trucking company as a new driver. Furthermore, it is not believable that, with only 9 months experience, you have been running an LLC but now want to become a company driver. It makes no sense unless you have a chapter 7 bankruptcy filing to back up your story. "The business went under," would be the only believable excuse, but then you have the problem that an LLC is public record. In the interview process, this scenario you've created would open up a can of worms where people who know the intricacies of business will ask you detailed questions, which require specific answers with esoteric knowledge. This would go down, if not to determine if you are lying, then just to shoot the breeze with you as "fellow business people." You don't want to do this. Besides, a quick google search will determine if a real LLC exists in your name. Any person who'd fall for this story would probably just hire you with unverifiable experience anyways.
