I only read the first page and last 3 pages so if I'm repeating anything that's been stated, my apologies. When the engine acts up, does it smoke at all? I ran into something similar on a 200 excavator a while back. Engine would miss/stutter and smoke a lot whenever they tried to use it. I had all kinds of codes for fuel injector faults, injector wiring faults, rail pressure faults, and fuel pump faults. Turned out to be an almost completely clogged strainer inside the fuel lift pump. When was the last time you replaced fuel filters?

My dad had the pre-Acert C15 in a truck back in the early 2000's that would constantly de-rate on hills. I can't remember if he had any knocks or noises but the de-rate issue was caused by the ECM reading the wrong ambient temperature. The fix the KW dealer had was a dummy plug and resistor that they plugged into the harness to fool the ECM into thinking it was close to 0C outside.

Fuel filters have been changed 3 times now since all this started, with no change. Ambient air temp is reading correctly according to cat et.
