Only time I see damage like that is when a cam goes out of time, over speed causing valve floating, smacking the pistons.

I replaced an ISB Cummins in a machine a few years ago. They claim overspeed from a failed turbo caused it to throw a rod right through the block and into the fuel pump. There were beach marks 3/4s of the way through on one of that rod's cap bolts though. So either way that engine was on borrowed time IMO.

Block heater cord shorted out earlier this year, luckily able to move another one away to stop the spread.
Some MX13 EPA 17 parts if you need them, been slightly heated

We referred to those as thermal events.
