I've driven 9, 10, and 13 speeds, and for what ever reason 10 was the hardest for me. I drove a 10 for 2 months and never could float or skip shift down. Maybe its just me; or maybe you, like me, learned on a 9 speed and are having a little trouble adjusting.

On the clutch thing, if its a modern truck you should be able to get it going in 2nd or 3rd at 78k gcw without touching the accl. Let the clutch out until the truck starts to pull forward. The rpms will dip a bit and then come back up. After that ease it out the rest of the way and give it some fuel. This has always been the smoothest way to start out for me. Be gentle with the clutch and she'll do whatever you like.

Are you wearing heavy boots? If so, then a lighter shoe might help give more "feel" of the pedal and its relation to the actual clutch and engine.

Also, is your seat adjusted so your legs are almost straight when on the pedals? That is most comfortable for me when on long runs. But for tight city driving I lower the seat a bit and get a little more bend in my knee.
