There was a sleeper called livalab that put the bed over the cab.
They used the freed up space for a bathroom and kitchen.
True custom has pic of one they stretched and rebuilt on there web site.

and question for you
do you use lower bunk in your scania or above windshield which is wider?
i can see lot of scania owners/drivers who have different arguments why they sleep on top or lower bunk
one my cousin fall down from top one
i can see lot of drivers who said that they never see anything such wide as topline upper bunk
some of them said same but that space between bad and roof is too small
my brother drives 1999 124 400 from 2002-2005 with cr19 comfort cab( same lower bunk as on topline) and truck have additional kit for bed which is placed on left wall behind driver during driving and this part is placed between seats while you sleep also seatback is folded down to make additional surface and those plastics on back could be removed so if you have time to convert it and dont need to move your truck during night this was quite nice solution -

here no one use shorter than 3.6m wheelbase ( for this one i think probably 5th wheel need to be closest possible to drive axle since you dont have large front axle weight , and to prevent space wasting -
those trucks were setup to run at the legal length limit of 25m and attain a carrying capacity of 36 pallets on the floor.
given it was all paper products and cereals there wasnt a lot of weight involved..
The drivers of these were never really all that happy cause its like a coffin up there, access through a hatch in the floor.
as for wheelbase, if i told you an exact figure it would be a guess, but they arent much longer than a regular car (hatch size) -
as much i find out only mercedes used in this
