Friend of mine has faults or injector 1,2,3 fmi 5, any known problems. Harness inside head looks ok not cracked or any thing abnormal. I will try to check wires from ecm to each injector, any other tips?

One injector is causing all three faults if the are always occuring u can try to change injectors one by one or the way i had this figured out a few times is made a d12 valve cover and cut off the middle of the top and i would have the engine running on idle and unplug injectors one by one or you can try to unplug then start so u can avoid all the oil splash usually its shorted injector causing all three injector faults

Take a temperature gun point it at each exhaust port. whichever one is the coldest is most likely the injector, or injectors that are bad. Temp gun is a lot cheaper then the Volvo Valve cover, you don't even have to pull the valve cover off for that test. We used to just put our hand but the exhaust in the old days, but now the temp gun makes it so much easier. Once you figure out which one is bad, you can pull that one replace it, put it back together and let the machine do its idle process and retime the engine to the new injector. We always just go ahead and pull all the injectors, replace the cups, o-rings, because it is most likely that a leaky o-ring or bad cup caused your injector to fail in the first place. The D12 injectors don't go bad that often unless the cups or O-rings are bad. Hope that helps.

I just found #2 Injector bad. The temperature gun would not helped me because ecm was turning off all 3 injectors. 1,2,3 have common power suppy. I disconnected one by one and only if I disconnect #2 truck runs on 5 cylinders if I disconnect 1 or 3 it runs on 3.

Also, check your fuel separator. I was having multiple injector problems, and while the injectors were indeed bad and had to be replaced, the fuel separator had to be replaced. Apparently that was causing water to get in the injectors and causing them to go bad.

^^^ good point, water and injectors don't mix, especially high pressure ones. Water can cause the tips to get blown off in a worst case scenario.
A good rule of thumb is to never use an emulsifier type fuel additive, always demulsify the water out of the fuel and drain it.
