Pull the shims and bottom pins.
Back the rear axle on to a curb or blocks.
Split the trailer and drive your front end on the deck.
Hook chains on both sides of the flip axle bumper (so it pulls over evenly) and hook to your truck tow hook. Put a rolled up tarp on the deck to cushion the axle when it comes over.
If your truck wont pull it over,you need to get the axle higher off the ground.
Or you can leave the neck on and run the chain over the neck and down to the truck to get a better angle and traction.
