OK, let's separate your questions. As to what you placed on the long-form you did right. Now if this history were to come back on you now you are covered. The ME has cleared you to operate a CMV. I have no real issues with this and you are 100% OK.
The felony question is not part of the medical per se. In the world of trucking, felonies can be problematic because there are certain aspects of this job you might never be able to get hired. As for as general trucking goes you are hirable as @Chinatown has stated. Time cures most felony problems, and 15 years is more than enough!

One of the things was when a chiropractor adjusted my neck and hurt me 10 or 12 years ago. I had to lay off my construction job for a few days and got an MRI. The orthopedic doctor said it wasn’t herniated. Rest and ice made it better. That was also the last time I let a chiropractor touch me.
No big deal, right? Well, this along with a 12 year old inguinal mesh surgery (turn your head and cough) created enough doubt that the people making the hiring decision simply said “next,” rather than asking for clarifying information.
So, be careful about what you tell them. They will probably find out about a felony drug conviction through public records, but your medical records are protected, just like your tax returns.
As for the long form, I disclosed the hernia surgery during CDL school, but apparently Cypress Truck Lines never saw it because they gave me an orientation date, and I showed up.
For their DOT medical, I disclosed it again it, but apparently Melton never saw it either, because they gave me an orientation date, and I kept my yapper shut this time. Throughout this eight week long process, no one ever asked about what was on that Cypress long form or the one from trucking school.
It would appear that the long form isn’t routinely requested or sent out. Maybe it’s a gray area, and trucking companies don’t want to run afoul of HIPAA laws? I’m just speculating at this point.
