I am going for my physical in a couple days. The problem is I am having trouble finding what exactly I need to do about paperwork pertaining to the physical. The place that does the physicals says that no paperwork is needed that everything is 100% electronic, the school I am going to said the paperwork is a long form with a card you keep off it. I had no luck finding the correct forms online either. I am in Pennsylvania if that helps.
The doctor has the long form and medical card. They will give you the medical card and they will send the long form to PA so it in The system or they give you the long form and you send it or whatever PA say you need to do. So PA jas proof you passed
Also need to take the card to the DMV and have added to your DL At least that's the rule here in FL, pretty sure its a Federal thing though
Thanks guys. I have called the school a couple times, but I don't think the counselor has a clue as to what he is talking about. They only offer the training one time a year to regular folks and the rest of the year is bought up by Sheetz for their driver trainees.
Far as I know everyone gets a copy when they leave the medical exam office,here in Virginia we have to take that copy down to dmv and they put it in the system and I believe the med exam office also does this electronically.
In my state of TN you don't have to go to DMV to show them your medical card or long form. You self-certify and email them a copy of the medical card.
