HIPPA does not cover anything beyond the handling of the medial data by the clinic. The long form and in general the US DOT physical data is not protected under HIPPA because we work in a Regulated Industry and enforcement agencies don't have to ask for investigational reasons go through the normal channels to obtain the DOT long form.
HIPPA also does not cover the carrier if they hold the medical long form or any medical data.
That said ... an examiner can request you provide the name of the treating physician for an issue or problem, like for sleep apnea or diabetes. There is no privacy release needed for this, but most clinics provide a release form per their policy.

I keep a folder with print outs of my medical appointments, take it to DOT Physicals and that way the Doc can look back through my papers and get an idea of my medical history. Makes things easier. Heard this idea years ago on sirius trucking channel.

The only way my statement makes sense is to see that it was an answer to this part of the OPs question. I know and understand that HIPAA does NOT cover the medical information that comes from physicals. However, I made a mistake and should have quoted that part. I apologize for the confusion!
