It's a good idea. I carry my contacts along with my list of medications, my last EKG, and a brief history of my health.

But would you suggest anything be added to the template?

It all depends on what you want them to know. The main reason I have the information is to inform the doctors on critical information they may need before any emergency contact can get to where I'm at. On your medical sheet you might want to list blood type, Any medication you can not take, and any specific problem areas you might have. The list just may save your life so you should make sure it's kept up to date.

ICE contacts list in your phone. First responders/hospital staff know to look there.

But that only works when you don't lock your phone to prevent accidental butt dials.

The SOS app is pretty cool IF you don't have a Windows Phone... I HATE Android, and do not want an iPhone.....

I'm on my third... Had an employer give me an Android for company use, and I couldn't get rid of it fast enough.

6 years working on the ambulance and 3 years working in the ER, 0 times did I pick up a patients phone and look for an ICE number. Maybe the cops do it but there's a real fine line as far as what we're even allowed to do there as calling anyone and saying anything gets into HIPAA territory.

I didn't read your sheet but in an emergency I want to know your medications and your allergies. I can tell you your medical history based on your meds so don't waste paper on that. Blood type doesn't really matter either as we will be giving you O neg until we get results back that tell us anyway but if you feel better go ahead and list it. Contact information is fine, eventually someone from the hospital will call but really all that matters is meds and allergies.
