Look for carbon tracks from exhaust leaking around the shaft of the exhaust brake to verify it's leaking there. Could be exhaust system leaking near there, too.
I haven't driven that truck since maybe 2011. -

Check your DOC behind the 7th injector. I had to replace mine(cost a few bucks) as it had some small almost invisible cracks in it that when I parked would open up and pour fumes in. Could not figure out where it was coming from, exhaust pipes were fine as well as everything else. Only saw some small carbon tracks then had it replaced. Might be your problem or might not. I am sure someone with more knowledge than I have will post the correct answer sooner or later but by then you might be dead from carbon monoxide poisoning.
