I used to get this about every 20-30k miles on my 2012 KWt660. Check engine came on and stayed on so normally delivered load and found a shop to work on it. The fault never said anything about the injectors just to see the manual etc. a couple of time they thought it was an ECU problem but ended up coming back on after time. Finally took it to a shop in lathrop to work on it and its been good since just had to spend a week of downtime waiting to get into the shop.. I thought it might have something to do with the fact I run team and it only seems to start a burnout when we are in traffic or coming to a stop...

that is what mine was doing. When comming to a stop or in traffic all of a sudden it would start regen. It was in the engine fault codes that I found that it was #1 DEF injector was not responding. KW had to take care of that.

your not talking about the amber regen light are you
when you have to do a manual regen -
I have a 2013 DD15, the amber engine shaped light came on, looked it up and it's the malfunction indicator lamp, there was a bad sensor, and the next week another one, but has been fine since then.
The shop refills our tanks, only problem with that is we don't know how much they are using. -
I have a 2012 Cascadia with the DD15.
Have put 60k miles on it, has 110k on it now.
Hasnt given me any issues.
I use between 6-10 gallons every 1500 to 2000 miles.
I almost never let it go below 3 bars/squares.
Last Fillup I went 2884 miles I added 8.61 Gallons. -
Have a 2012 Pete 386 . I got it with 22 miles and about to turn 100k. I've yet to have any problems with the DEF system. Hell, the only issues I have had at all was a axle flange leak and frayed wire on the TK APU.

also, are these new engines any good with DEF? Do you still regen with def? I hear its the 07-09 engines are the real ###### ones cuz they used EGR -
just curious about def fuel in general... are there different grades? stupid question i know, but it seems if i fill my def tank with swifts def fuel, i get crappy miles out of that tank, i do find that if i use sap bros. def fuel i seem to get better miles with the same weight of loads... i try not to fill my def at swift yards as they take the fuel cost out of yer maint. acct. and again i get crappy miles.

The only time that I use the DEF at the yards if almost mt, and then I only put in about 5 gals. I know that I pay more outside, but then again, I still never fill the tank. I usually just put in enough to last 1 week. The last time that I filled, I did get at the yard. Told the guy, only wanted about 5 gals. Guess that he did not hear me, and I noticied, less than 3k miles, and I am down to 1/4. I thought that they are the same solution, but remember, it is the Swift yard so who knows.
