DMV says that a driver must prove that they held a CDL in the past to get a new CDL. I can prove that I did have a CDL by furnishing my driving record. I last held a CDL in 2011 but had not driven commercially since 2009. I have 7 plus years of over the road experience also.
With that said, am I going to be required to attend a truck driving school regardless of the above? And carry a permit for 30 days? OR Both?

Well, there is no requirement in ANY state that you have to go to driving school to get a CDL...just take the written,skills, and road test and you are good to go if you find someone to teach you. There is also no required time to have a permit (at least in my state) before you can get your CDL. You could get a permit on day one, test on day two and have your CDL that day if you are that good.
Now if you are asking if you have a CDL, then went to a class C, and wanna get your CDL back...that is up to your state on requirements to reissue weather you have to take the written, skills, road test..or none at all. Keep us posted, I am curious.

you can get your cdl back , you might be required to take a refresher course or go back to truck driver school depending on who your trying to get hired on with..... I let my cdl go in 1990 back when you had to take all these endorsement tests. back then all i had to do was go take the writtens and i was grandfathered , i didn't do that.... I recently got mine back , I had to do the whole program. PITA!!!!!!! I went to calif dmv took the tests and rented a truck and took the skills test and now I am driving a super ten dump .
( Trucking schools hate guys like me , I had a bunch trying to blow smoke up my butt, telling me the law requires 160 hours of trucker training at $8000 a pop. I did it on my own, as i didn't need to learn how to drive a truck again, I just needed a truck to do the skills test in.)
I also have the option to go back otr, which I am seriously looking at due to the money in otr vs. Local driving....
What your gonna run up against the most is recent experience. Every company has different requirements.
Personally I think that is bulls--t....
Driving is like riding a bike or having sex you don't forget how,
and if you aint got laid in a while, I doubt 99.99999999999999999 % of us forgot how, due to lack of recent experience....
A large percentage of local companies will hire a guy with good previous experience, just for that reason. So you have that option to explore....
For refresher course 40 hour it was somewhere around 3 grand ,

Did you have your CDL suspended at one time?

in utah, you let it lapse for 90 days. you have to do it all over again. and it's 14 days for a permit.
as for being off the road. i got told by JCT you needed to know how to drive around the country. i told them that's a pitiful excuse. maps will show you the way. and gps takes you anywhere you wanna go.
