“In an attempt to protect both public safety and the Deputy himself, the sheriff gave him only one bullet which he had to carry in his shirt pocket. Barney's attempt to get that bullet into his gun was the cause of many comedic moments in the show.” :)

As long as you stay at least one night in a Holiday Inn to cancel out the hospital stay the Dot won’t care.

Unless you have obvious scars from surgery, have been legally buying prescription pain medication, they DOT doctors only know what you write on the DOT paperwork. Legally they want the name, date, location, and supporting paperwork for ever hospital stay since birth. Practically speaking, if you forgot to document your tonsils being removed 24 years ago, they won't hassle you. The DOT exam doesn't even test for illegal drugs, just sugar in your urine. The DOT doctor isn't your partner in health, he's not your drinking buddy or counselor. He's the guy that can take away your income if you say the wrong thing or put it on the DOT paperwork. Many DOT doctors will interpret any talk by you about snoring or sleeping problems as an admission you have sleep apnea and need an expensive sleep study and a CPAP machine. Answer the question they ask, and nothing more. Assume anything said to any employee is being said to the doctor. They can't read your mind. There is no central computer with all of your medical history. Don't shoot yourself in the foot by confessing to something medically unimportant.

You are correct you should never lie on any area of the physical. If it is as minor surgery you should not have to worry. If it was something like one doctor telling you to do a sleep study, you do and don't like the results of that sleep study so you go to another doctor and lie about it ... if you have an accident and that lie comes to the surface you could get jail time and fines. This next piece is from the internet asking a question like yours.

No, you cannot go back to a doctor and correct a lie on a CDL physical exam, because lying is considered fraud and can lead to serious penalties. The correct action is to get a new physical from a different Certified Medical Examiner (CME) and provide a fully honest and complete medical history. The Federal Motor Carrier Safety Administration (FMCSA) requires all previous medical history to be reported accurately on the new form.
