The rates are based upon whether your S/O is a company driver or L/O. Also, whether or not he's been there less than a year or more than a year. There's a "low pay out" option and a "high pay out" option. And to take it one step further, there's also associate only coverage, associate and spouse, associate and children, and family coverage.
